About the Job

The Clinical Case Manager RN will work closely with a diverse population of patients, assessing their current health status, identifying gaps or barriers in treatment plans, and providing patient education to assist with self-management. The successful candidate will also interact with Medical Directors on challenging cases, coordinate care for members, and make referrals to outside sources as needed.

About You
  • You have an active, unrestricted RN license in the State of New York.
  • You have 3+ years of RN experience in a hospital setting, acute care, direct care, or as a Case Manager at an insurance company.
  • You are proficient in navigating a Windows environment, using MS Outlook, creating and editing documents in MS Word, and talking and typing simultaneously.
  • You are willing or able to work Monday - Friday: 3 days/week 8 am - 5 pm and 2 days/week 11 am - 8 pm.
  • You are willing or able to work onsite in our Cheektowaga, NY office for the first 6-12 months.
