Community Case Manager

Families seeking support can rely on our Community Case Manager to provide essential services, including recruitment, data management, and record-keeping.

Responsibilities:

  • Screen new children's applications, conduct interviews, and complete the enrollment process.
  • Assign children to classrooms and maintain accurate records, including health data tracking.
  • Prepare data for computer input, perform data entry, and generate reports to support program success.
  • Conduct health screenings, home visits, and make referrals to community agencies as needed.

Requirements:

  • Associate's degree in Social Work, Social Services, Sociology, Human Services, Psychology, or Early Childhood Development, or equivalent experience.
  • Intermediate computer skills and knowledge of Microsoft Office required.
  • Training in nursing/health-related areas or education in child development preferred.

Compensation:

Estimated salary: $15.00 - $15.30 per hour
