Position Overview:
As a vital member of the healthcare team, the Registered Nurse will engage in comprehensive case management practices, ensuring adherence to established standards of care.

Key Responsibilities:
  • Case Management Program Participation: Actively engage in all aspects of the Case Management Program (CMP), ensuring compliance with established case management standards.
  • Nursing Expertise: Provide specialized nursing knowledge throughout the case management process, including assessment, planning, implementation, coordination, and monitoring.
  • Strategy Development: Formulate and execute local strategies utilizing various case management methods, including inpatient, outpatient, and telephonic approaches.
  • Tool Development: Create and implement tools to enhance case management efficiency, such as patient identification and assessment tools, clinical practice guidelines, and databases for community resources.
  • Collaboration: Work closely with community agencies and organizations to maintain effective communication and support.
  • Data Management: Collect and document patient care data accurately to ensure quality care delivery.
  • Treatment Planning: Develop comprehensive treatment plans that encompass preventive, therapeutic, rehabilitative, and psychosocial interventions to promote optimal wellness.
  • Patient Education: Provide essential healthcare instructions to patients and caregivers based on identified learning needs.

Additional Responsibilities:
  • Utilize advanced information technology and management tools to enhance patient engagement and experience.
  • Facilitate communication with patients through secure messaging platforms to improve care coordination.
  • Coordinate multidisciplinary teams to address the healthcare needs of patients effectively.
  • Establish and maintain policies for home health assessments and outcome measures.
  • Prepare analytical reports and conduct evaluations to improve service delivery.
  • Ensure compliance with regulatory standards and maintain accurate patient care data management.
  • Collaborate with team members to set and achieve patient-specific goals.
  • Provide nursing consultation and advice both in-person and via telecommunication.

Qualifications:
  • Education: Bachelor’s degree in Nursing from an accredited institution; a Master’s degree in Nurse Case Management is preferred.
  • Experience: Minimum of 5 years of experience as a Registered Nurse Case Manager within the last 7 years.
  • Certification: Current certification from accredited organizations is required.
  • Licensure: Must hold an active and unrestricted Registered Nurse license.
  • Security Clearance: Ability to pass a government background check is necessary.
  • Life Support Certification: Current AHA or ARC BLS Healthcare Provider certification is mandatory.
