About the Role

We are seeking a highly motivated Social Worker in Adult Psychiatry to join our team. The ideal candidate will be a compassionate and skilled professional with a Master's degree in Social Work and experience in providing social work treatment for patients in various settings. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Job Summary

As a Social Worker in Adult Psychiatry, you will be responsible for assessing the biopsychosocial needs of patients and their families, developing individualized plans of care, and providing clinical interventions to address identified needs. You will work closely with the multidisciplinary team to provide care coordination, crisis intervention, and supportive counseling services.

Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments of patients and their families, identifying strengths, dysfunctional factors, and establishing discharge/transition of care plans.
  • Develop and implement individualized plans of care, utilizing evidence-based practices and current research to inform treatment decisions.
  • Provide clinical interventions, including crisis intervention, supportive counseling, and case consultation services, to patients and their families.
  • Collaborate with the multidisciplinary team to provide care coordination, ensuring seamless transitions of care and effective communication.
  • Document patient progress, treatment plans, and discharge/transition of care plans, maintaining accurate and up-to-date records.
Requirements
  • Master's degree in Social Work from a CSWE-accredited college or university.
  • 1-3 years of experience in social work practice, preferably in a healthcare setting.
  • Knowledge of coordination of care/discharge planning, crisis intervention, and supportive counseling.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
