Job Summary:

Children's Wellness Coach working in collaboration with the Boys and Girls Clubs of the Valley at assigned sites. Job involves interacting with children, staff, and families, utilizing the Positive Action prevention model and other clinical approaches based on Club needs.

Key Responsibilities:
  • Conducts engagement process with families to identify natural/formal supports
  • Facilitates Child and Family Team/Service Plan meetings to create individualized service plans
  • Ensures families understand the behavioral health process and participate in service plans
  • Coordinates and schedules meetings with natural, formal supports, and system stakeholders
  • Submits referrals to clinical outpatient services
  • Updates electronic medical records
  • Completes strength assessments and crisis/safety plans
  • Develops positive supportive relationships with consumers and stakeholders
Work Environment:

This position is co-located within Boys and Girls Club of the Valley locations. Bilingual in Spanish is a plus (Additional Language Differential Pay Available). Full-Time Employed: 40 hours/week. M-F: 11a-7p. Starting at $ 19/hr.

Qualifications:
  • GED/High school diploma with 1.5 years of behavioral health experience, or
  • Associate’s degree in medical or behavioral health, or
  • Bachelor’s degree (not in behavioral health) with 1 year of behavioral health experience, or
  • Bachelor’s degree in behavioral health, or
  • Master’s degree or higher in behavioral health
  • Preferred: Bilingual proficiency in English and Spanish
  • Computer skills: Microsoft Word, Excel, Outlook, electronic medical record
  • Valid AZ Driver License
  • Valid Arizona Fingerprint Clearance card (Level 1) or apply within 7 working days
  • Interpersonal skills, good communication skills, and basic understanding of the DSM-V preferred
  • Certified Case Manager strongly preferred
  • Must have or attain CPR/First Aid certification within 60 days
  • Must pass background check, TB test, and other pre-employment screening
