MDS Coordinator Opportunity

We are seeking a skilled MDS Coordinator to join our team at Park Place Nursing & Rehabilitation Center in Tyler, TX.

Key Responsibilities:
  • Maintain accurate and up-to-date MDS/Care plan schedules for interdisciplinary teams
  • Coordinate preparation of MDS and Care Plans in accordance with regulatory requirements
  • Track Medicare resident benefit days and validate daily Medicare census
  • Ensure accurate billing and preparation of Medicare denial letters
  • Participate in ongoing evaluation of documentation required for Medicaid reimbursement
Requirements:
  • Current RN or LVN license in the state of Texas
  • 2+ years of experience in skilled nursing facilities with previous experience in completing MDS's and Care Plans as an MDS Coordinator
  • Demonstrated proficiency in RAI/PPS processes
  • Strong attention to detail, documentation, and resident assessment skills
  • Good communication, clerical, organizational, and computer skills
