Job Summary:

As a Licensed Practical Nurse (LPN) with Eden Health, you will provide skilled nursing care to patients in their homes throughout the Vancouver/Portland Metro Area. Responsibilities:
  • Provide skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload.
  • Review caseload with supervising RN on a regular basis as established by best practice and/or State guidelines.
  • Obtain necessary physician orders and collaborate with the physician as the patient's needs dictate.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participate in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.
  • Use equipment and supplies effectively and efficiently.
  • Provide appropriate pain/symptom management. Evaluate patient's response to treatments/medications.
  • Actively participate in Quality management program.
  • Complete accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.
Requirements:
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice.
  • Experience as an LPN/LVN commensurate with one of the following preferred:
    • One year experience in an acute care setting within the last 24 months;
    • Six months home care experience within the last 12 months preferred.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass criminal background check and national sex offender clearance.
