Job Summary:

We seek a compassionate Registered Nurse to join our Home Health team as a Patient Care Coordinator. The ideal candidate will deliver personalized care to patients in their homes, ensuring seamless coordination between healthcare professionals.

Responsibilities:

  1. Conduct comprehensive in-home patient assessments and develop tailored care plans.
  2. Provide skilled nursing care to patients according to physician orders.
  3. Monitor patients' vital signs and symptoms, reporting any changes to the healthcare team.
  4. Educate patients and their families on disease management and treatment plans.
  5. Collaborate with healthcare professionals to ensure comprehensive patient care.
  6. Maintain accurate and up-to-date medical records.

Qualifications:

  • Valid RN license in the state of practice.
  • At least 1 year of nursing experience, preferably in home health or a related field.
  • Experience in pediatric care, case management, utilization management, infectious disease care, acute care, managed care, or critical care is an asset.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in medical documentation and electronic health records.
