We are seeking a dedicated and compassionate Director of Nursing & Clinical Operations to join our team at Hospice Care Partners.

As a key member of our leadership team, the Director of Nursing & Clinical Operations will be responsible for ensuring quality patient care in the home setting. This role requires strong administrative skills, excellent communication abilities, and a passion for delivering exceptional patient outcomes.

Key Responsibilities:

  • Administrative oversight of clinical staff, including RN Case Managers, Nurse Aides, Social Workers, Chaplain/Bereavement Coordinators, and Volunteers
  • Maintenance of a regular caseload as dictated by the needs of the agency
  • Professional coordination of care with the interdisciplinary group, patient, family, and referring agencies
  • Monitoring and analysis of QAPI issues and direction of clinical staff regarding agency policies and procedures

Requirements:

  • 3 years of hospice case management experience and 1+ years of management experience in hospice
  • TX RN license that is current and unencumbered
  • Reliable transportation with current insurance coverage
  • Current CPR certification
  • Current TB test with negative result or equivalent
  • Able to see patients and conduct regular supervisory visits within our service area (Arlington, Fort Worth, Mid-Cities, Mansfield, South Dallas, and Ennis)
  • Able to work 1 weekend of On-Call per month
