Skilled Nursing Care Specialist

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Skilled Nursing Care Specialist. In this role, you will provide high-quality patient care and collaborate with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities:

  • Provide one-to-one patient care in a home environment
  • Execute physicians' prescribed plans of care and document care provided
  • Administer prescribed medication, treatments, and therapies
  • Patient assessments, coordination of care
  • Health promotion, teaching, and training of family members

Requirements:

  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
