Licensed Practical Nurse LPN Care Coordinator

Aveanna Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to patients in their homes. As an LPN, you will be responsible for delivering and coordinating quality patient care in compliance with physician orders.
About the JobThe ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently. You will be required to continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
Responsibilities- Deliver and coordinate quality patient care in compliance with physician orders
- Continuously observe and assess patient condition and care needs
- Report changes to the supervisor and/or physician as appropriate
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
To be successful in this role, you must have:
- A graduate degree from an accredited school of nursing
- A current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
We prefer candidates with:
- Six months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
This position requires:
- The ability to speak, write, read, and understand English
- The ability to travel
- The ability to lift 50 pounds
- The ability to sufficiently reposition patients and move equipment without assistance
- Prolonged walking, standing, bending, kneeling, reaching, twisting
The estimated annual salary for this position is $$64,600 - $85,800, depending on location and experience. This salary range is based on national averages and may vary depending on the specific location and other factors.
