Job Title: Licensed Practical Nurse - Home Health Specialist

About the Role:
We are seeking a skilled and compassionate Licensed Practical Nurse to join our interdisciplinary team at Providence VNA in Spokane, WA. As an LPN in Home Health, you will have the unique opportunity to provide high-quality care to patients in their own homes, fostering rewarding relationships and exercising independence and flexibility in your career.

Estimated Salary: $55,000 - $65,000 per year (based on experience)

About the Job:
As an LPN, you will work alongside an interdisciplinary team to provide whole-person quality care to patients in need. You will be empowered by our shared governance model that includes nursing staff in the process of creating practice standards and expectations.

Responsibilities:
* Provide direct patient care and develop individualized care plans
* Collaborate with other healthcare professionals to coordinate patient care
* Conduct comprehensive assessments and monitor patient progress
* Educate patients and families on disease management and self-care techniques

Required Qualifications:
* Graduate of an accredited Licensed Practical/Vocational Nurse program
* Washington State LPN licensure or eligibility
* National Provider BLS certification within 30 days of hire
* Reliable transportation and valid driver's license

Preferred Qualifications:
* 1 year recent hospital, nursing home, or home health experience
