About the Role

We're seeking a highly skilled Licensed Practical Nurse (LPN) to join our team. As an LPN at Oak Street Health, you'll play a vital role in delivering high-touch, primary care to our patients. Your responsibilities will include assisting RNs with patient care, collecting health data, administering medications, and providing telephonic support to patients.

Requirements
  • A current state license to practice as an LPN
  • CPR Certification required
  • EMR experience
  • Strong communication skills and customer service orientation
  • A problem-solving orientation, accountability, and a flexible and positive attitude

In this role, you'll have the opportunity to make a meaningful impact on the lives of our patients. We offer a competitive salary of $65,000 - $80,000 per year, depending on experience, plus benefits, including paid vacation, sick time, and investment/retirement 401K match options.
