Clinical Liaison

The Terraces of Phoenix, a HumanGood community, is hiring a full-time Clinical Liaison to join its Skilled Nursing team. As a key member of our team, you will be responsible for generating business from referral sources to meet the community's revenue and census goals.
This role involves developing relationships with community hospitals, physician offices, community health clinics, and other community outreach programs. You will contribute to the success of HumanGood by coordinating external professional relationships for the community, with a focus on Skilled Nursing components of senior living, to achieve the highest possible occupancy within budgetary and regulatory guidelines.
Key Responsibilities:
- Develop and maintain relationships with referral sources to meet revenue and census goals
- Conduct outside sales and marketing activities to promote the community's Skilled Nursing services
- Collaborate with internal teams to ensure seamless care transitions and high-quality patient outcomes
- Stay up-to-date with industry trends, best practices, and regulatory requirements
Requirements:
- Two or more years of experience in a related field, preferably in long-term care or geriatric settings
- Current or prior Admissions, Clinical, or Post-Acute Liaison experience within a skilled nursing facility preferred
- Clinical license such as RN, LPN, RT, IT, or SLP preferred
- Bachelor's Degree, Medical or Business education, or training preferred
- Valid driver's license
What We Offer:
- Competitive salary range: $65,000 to $85,000 (DOE) plus commission
