Clinical Liaison

The Terraces of Phoenix, a HumanGood community, is seeking a skilled Clinical Liaison to join its Skilled Nursing team. As a key member of our team, you will be responsible for generating business from referral sources to meet our revenue and census goals.
Key Responsibilities:
- Develop relationships with community hospitals, physician offices, and community health clinics to drive referrals and admissions.
- Coordinate external professional relationships to achieve the highest possible occupancy within budgetary and regulatory guidelines.
- Collaborate with the Skilled Nursing team to ensure seamless care transitions and patient flow.
Requirements:
- Two or more years of experience in a related field, preferably in a long-term care setting.
- Current or prior Admissions, Clinical, or Post-Acute Liaison experience within a skilled nursing facility preferred.
- Clinical license such as RN, LPN, RT, IT, or SLP preferred.
- Bachelor's Degree, Medical or Business education, or training preferred.
- Valid driver's license.

- A competitive salary range of $65,000 to $85,000 (DOE) plus commission.
