Job Title: Clinical Liaison

The Terraces of Phoenix, a HumanGood community, is seeking a skilled Clinical Liaison to join its Skilled Nursing team. As a key member of our team, you will be responsible for generating business from referral sources to meet our revenue and census goals.

Key Responsibilities:

  • Develop relationships with community hospitals, physician offices, and community health clinics to drive referrals and admissions.
  • Coordinate external professional relationships to achieve the highest possible occupancy within budgetary and regulatory guidelines.
  • Collaborate with the Skilled Nursing team to ensure seamless care transitions and patient flow.

Requirements:

  • Two or more years of experience in a related field, preferably in a long-term care setting with geriatric residents, admissions, and marketing.
  • Current or prior Admissions, Clinical, or Post-Acute Liaison experience within a skilled nursing facility is preferred.
  • Clinical license such as RN, LPN, RT, IT, or SLP is preferred.
  • Bachelor's Degree, Medical or Business education, or training is preferred.
  • Valid driver's license is required.

  • Competitive salary range: $65,000 to $85,000 (DOE) plus commission.
