Position OverviewGeneral Summary
Under minimal supervision, this role involves delivering an advanced level of comprehensive health services to a designated patient demographic in partnership with medical personnel. Adhering to established nursing and medical protocols, the incumbent will provide specific care instructions to nursing and support staff.
Key Responsibilities
The following responsibilities outline the essential functions of this position. This list is not exhaustive and may be subject to change.
- Conducts thorough medical assessments, diagnoses, treatments, follow-ups, consultations, and health education tailored to the patient's age and circumstances.
- Collects patient histories and formulates care plans, ensuring thoroughness and accuracy in documentation.
- Requests, acquires, and interprets relevant diagnostic evaluations.
- Develops, orders, and records suitable treatment strategies, including medication prescriptions based on a detailed analysis of health history and diagnostic findings.
- Communicates care plans effectively with relevant healthcare providers, including collaborating physicians, primary care providers, nursing staff, and case managers.
- Engages with patients and their families to explain care plans, disease processes, new treatment options, and medication schedules.
- Delivers medical attention to patients experiencing illness or injury, adhering to accepted care standards.
- Provides health education to patients and families as necessary.
- Records medical observations and care provided in the patient's clinical documentation, including laboratory and imaging study results.
- Educates patients on health maintenance practices and supplies educational resources as needed.
- Collaborates with physicians and other healthcare team members regarding patient health needs, outcomes, and the effectiveness of care plans.
- Initiates appropriate communication with other healthcare providers as necessary.
- Safeguards the privacy and security of all confidential health information, disclosing only what is essential for job performance.
Education and Experience
- Master of Science degree with completion of an accredited Nurse Practitioner Program is required.
- Current state licensure as a Nurse Practitioner is required.
- Current state licensure as a Registered Nurse is required.
- Current BLS certification or obtained prior to delivering direct patient care is required.
- Experience is preferred but not mandatory.
- DEA and CDS eligibility is required.
Knowledge, Skills, and Abilities
- Demonstrates knowledge and skills necessary to provide care appropriate to the patient population served, including an understanding of growth and development principles across the lifespan.
- Exhibits strong verbal and written communication skills for effective interaction with patients, families, and healthcare staff.
- Maintains current training in hospital safety and other mandatory areas.
- Ensures immunizations are up to date as recommended for the practice area.
Patient Safety Commitment
Prioritizes patient safety in all job functions and participates in departmental patient safety initiatives.
- Takes corrective actions to address observed risks to patient safety.
- Reports adverse events and near misses to the appropriate management authority.
- Identifies potential risks in processes and communicates these to relevant personnel.
