Family Advocate

We are seeking a highly skilled and compassionate Family Advocate to join our team at Nest Health. As a Family Advocate, you will play a vital role in providing direct patient care and support to families in their homes.
Job Summary:The Family Advocate will work closely with our interdisciplinary care team to provide comprehensive care to patients, including medical assistance, health education, and social support. This is a unique opportunity to make a meaningful difference in the lives of families and contribute to the growth and success of Nest Health.
Key Responsibilities:- Provide direct patient care and support in the home setting, including medical assistance, health education, and social support.
- Work closely with the interdisciplinary care team to develop and implement individualized care plans.
- Conduct home visits to assess patient needs and provide support and education.
- Collaborate with healthcare providers and other team members to ensure seamless care coordination.
- Participate in outreach efforts to engage with patients and families, promoting health and wellness.
- Document patient information and services provided in accordance with Nest Health's electronic medical record system.
- Successful completion of an accredited Medical Assistant program.
- Fluency in English is required for this position.
- High School Diploma or equivalent required; 1-3 years of relevant experience preferred.
- US work authorization.
- Vital Signs and phlebotomy experience.
- EMR Systems experience.
- Experience in counseling, community work, or community health activities in a government agency or community-based organization engaged in providing services to the public.
