Key Responsibilities:
  • Assessment and Goal Setting:
    • Utilizes suitable evaluation techniques to determine the capabilities and limitations of patients across all age groups.
    • Conducts and interprets initial assessments to evaluate the patient's functional status in areas such as mobility, strength, endurance, and pain management.
    • Establishes measurable and functional short-term and long-term objectives.
    • Completes foundational evaluation competencies.
  • Patient Care Provision:
    • Designs effective treatment plans for patients of all ages based on physician referrals, assessment outcomes, and patient goals.
    • Administers daily therapeutic interventions, ensuring age-appropriate modifications as required.
    • Completes all necessary competency evaluations related to patient care.
    • Identifies the need for group therapy and coordinates scheduling as needed.
    • Engages with team members, healthcare professionals, and family members to share information that enhances patient care.
    • Selects and instructs patients on the use of assistive devices.
  • Education for Patients and Families:
    • Provides tailored education to patients and their families, addressing learning preferences and barriers.
  • Discharge Planning:
    • Collaborates with the healthcare team to assess patient readiness for discharge, planning for home care, equipment needs, and ongoing services.
  • Documentation:
    • Ensures all documentation is completed in accordance with departmental policies.
  • Operational Duties:
    • Maintains a productive schedule, averaging 6.5 hours of direct patient care daily, and communicates any scheduling changes to supervisors.
    • Handles additional responsibilities such as billing for services rendered and maintaining a clean work environment.
    • Participates in departmental quality improvement initiatives and adheres to safety protocols.
    • Actively engages in departmental meetings and continuing education opportunities.
  • Supervision of Assistive Personnel and Students:
    • Delegates tasks to assistive personnel while overseeing patient treatment plans.
    • Mentors physical therapy interns as requested after gaining sufficient clinical experience.
Licensure:
  • Must hold a valid Physical Therapist license.
Education/Experience Required:
  • Master's or Doctorate degree in Physical Therapy from an accredited institution (or a Bachelor's degree for graduates prior to 2002). Must meet annual stroke education requirements for therapists working with stroke patients.
Knowledge, Skills & Abilities Required:
  • Strong interpersonal and communication skills.
  • Must be licensed in the state of Illinois.
Physical Requirements and Working Conditions:
  • Availability to work approximately one weekend per month or evenings as required. Must be able to lift varying weights as necessary, utilizing patient handling equipment when appropriate.
