Key Responsibilities:
- Assessment and Goal Setting:
- Utilizes suitable evaluation methods to assess the capabilities and limitations of patients across all age groups.
- Conducts and interprets initial assessments to determine the patient's functional status in various areas including mobility, strength, endurance, and more.
- Establishes both short-term and long-term goals that are functional and measurable.
- Completes entry-level evaluation competencies.
- Patient Care Provision:
- Designs tailored treatment plans for patients of all ages based on physician referrals, evaluation outcomes, and patient/family objectives.
- Administers daily treatments, ensuring they are age-appropriate and modified as needed to meet the evolving needs of the patient.
- Successfully completes all competency assessments related to patient care.
- Identifies the need for supplemental group therapy and coordinates scheduling as necessary.
- Engages with team members, other professionals, and family members to share information that aids in providing optimal patient care.
- Selects appropriate assistive devices for patients and instructs them on their maintenance and usage.
- Education for Patients and Families:
- Delivers educational support to patients and their families based on identified needs, addressing learning readiness and barriers.
- Discharge Planning:
- Collaborates with team members to assess when a patient is ready for discharge, planning for home treatment, equipment needs, or continued services.
- Documentation:
- Ensures all documentation is completed in accordance with departmental policies.
- Operational Duties:
- Maintains a productive schedule, averaging 6.5 out of 8 hours per day dedicated to direct clinical care, and informs supervisors of any schedule changes.
- Handles billing for services rendered and maintains a clean work environment while participating in departmental committees and program development.
- Contributes to the continuous improvement of service quality by engaging in critical function measurement and process improvement teams.
- Adheres to safety and infection control protocols related to patient care and promptly reports any issues to supervisors.
- Actively participates in departmental meetings, in-services, and continuing education opportunities.
- Supervision and Delegation:
- Delegates patient care tasks to assistive personnel while maintaining responsibility for the treatment program and goals.
- Supervises physical therapy interns as requested after completing one year of clinical experience.
- Must hold a valid Physical Therapist license.
- Master's or Doctorate degree in Physical Therapy from an accredited institution (or a bachelor's degree for graduates prior to 2002). Must meet annual stroke education requirements for working with adult stroke patients.
- Strong interpersonal and communication skills.
- Licensed to practice in the state of Illinois.
- Availability to work approximately one weekend per month or evenings as needed. Capable of lifting weights ranging from 10 to 100 pounds depending on the situation.
- For patient lifts exceeding 35 pounds, appropriate equipment and assistance are expected to be utilized.
