Chronic Care Manager

Northwind Pharmaceuticals is seeking an experienced Chronic Care Manager to join our team. As a key member of our Clinical Blueprints team, you will be responsible for managing patient cohorts, monitoring eligibility, and identifying high-risk patients.
The ideal candidate will have a strong background in coaching, fitness, or nutrition, with a focus on chronic care management. A Registered Dietitian (RD) or Certified Diabetes Educator (CDE) certification is highly desirable.
This is a hybrid onsite and remote position that requires strong organizational and time management skills. If you are a detail-oriented individual with excellent communication skills, we encourage you to apply.
Job Responsibilities:- Patient Cohort Management: Manage patient cohorts, monitor eligibility, and identify high-risk patients.
- Program Development: Collaborate with multidisciplinary teams to develop and implement Clinical BlueprintsTM programs.
- Documentation: Accurately document patient interactions and outcomes using our proprietary software system.
- Communication: Communicate effectively with healthcare providers, clinics, and employers to ensure seamless program execution.
Estimated Salary: $65,000 - $85,000 per annum based on experience and qualifications.
