About the Position
This is a Nurse Practitioner opportunity at Abode Care Partners, where you will work in various settings, including homes and assisted living facilities, providing compassionate, evidence-based medical care to seniors and patients with complex needs. The position involves planning, directing, and evaluating patient medical needs, examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses, collaborating with caregivers and other healthcare professionals in providing interdisciplinary care, and maintaining a comprehensive plan of care.

Responsibilities
The responsibilities of this Nurse Practitioner role include:
  • Providing evidence-based medical care by planning, directing, and evaluating patient medical needs
  • Examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
  • Ordering and interpreting laboratory and diagnostic testing as needed
  • Collaborating with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
  • Providing referrals to other healthcare practitioners when necessary
  • Maintaining patient medical records

Requirements
To be successful in this role, you will need:
  • An active state Nurse Practitioners license
  • A current Nurse Practitioner National Board Certification
  • An active state DEA licensure
  • A Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
  • Proficiency in medical instruments and equipment
  • Strong knowledge base of HIPAA and OSHA regulations
  • Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
  • A licensed driver with valid automobile insurance
  • The ability to engage in moderate physical activity
