The Patient Care Specialist - Chronic Kidney Disease Management role involves providing exceptional care to patients in an outpatient dialysis setting. With a focus on building long-term relationships, the ideal candidate will oversee patient observations, operate medical equipment, and collaborate with a supportive team.

In this challenging yet rewarding position, you will be responsible for delivering care to patients who are often dealing with multiple co-morbidities. Your unique treatment plans and broad range of nursing skills and knowledge will make a significant difference in their lives.

You will work closely with PCTs to conduct patient observations, measuring vital signs and machine setup. In return, you can expect a positive and fast-paced environment where your head, heart, and hands are put to use every day.

Salary/Wage Range:

$60,000 - $80,000 per year, depending on experience and qualifications.
