Key Responsibilities- Conduct comprehensive patient assessments, including history taking and physical examination, to identify patient needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to coordinate patient care, including ordering diagnostic tests and medications, and consulting with physicians and other healthcare professionals as needed.
- Develop and implement patient education plans to promote health literacy and self-management of chronic conditions.
- Participate in quality improvement initiatives to enhance patient outcomes and safety.
- Stay current with evidence-based practice guidelines and regulatory requirements to ensure compliance and optimal patient care.
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
- Current licensure as a Registered Nurse (RN) in the state of California.
- Current certification as a Nurse Practitioner (NP) in Acute Care or a related specialty.
- Minimum of 6 months of experience as a Nurse Practitioner in an acute care setting.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Experience working in a Kaiser Permanente facility.
- Knowledge of electronic medical records and other healthcare technologies.
- Ability to work flexible hours, including evenings and weekends.
