Job Description

We are seeking a skilled Intake Case Manager to join our team at Puzzle HR. As a key member of our community services team, you will be responsible for providing compassionate and respectful care to clients living with HIV.

Key Responsibilities:

  • Represent Puzzle HR in a professional and meaningful way as the first point of contact with clients seeking services.
  • Complete full intakes with new clients, including biopsychosocial assessments and initial plans of care that support clients in connecting to necessary services.
  • Assist clients newly diagnosed with HIV in seeking medical care and attending appointments within 30 days of diagnosis.
  • Provide comprehensive case management services to help clients address the complex challenges associated with living with HIV.
  • Meet regularly with clients in the office, in their homes, and in the community.
  • Conduct ongoing biopsychosocial assessments on all clients on a semiannual basis.
  • Assist clients with connections to medical, social, community, financial, and other needed services.
  • Assist clients in developing realistic action steps to achieve their goals.
  • Provide support services and referrals to clients who may be socially isolated.
  • Provide transportation to clients as needed to access medical, community, and agency services.
  • Advocate for clients within the agency and in external settings, such as medical providers and other agencies.
  • Build relationships with medical and social services providers to increase continuity of care.
  • Complete all documentation in an appropriate manner and in a reasonable timeframe according to agency standards.
  • Practice appropriate boundaries with clients and with work to ensure excellent services to clients while preserving self-care.

Requirements:

  • Bachelor's degree (preference for Social Work or other Human Services) - Must provide verifiable proof of degree.
  • Minimum of two years' experience in community-based social or health care services - a supervised BSW/MSW internship may count for one year.
  • Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology.
  • Valid Driver's License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work.

Preferred Qualifications:

  • Bi-lingual abilities preferred.
