Medication Management Counselor, LPT, LVN

The Medication Management Counselor role is essential within our organization, focusing on providing support to residents in their medication-related needs.
KEY RESPONSIBILITIES
- Perform comprehensive nursing evaluations, including health history and vital signs during the intake process.
- Supervise residents in their self-administration of prescribed medications.
- Ensure all residents undergo weight assessments upon entry, alongside vital sign checks.
- Facilitate connections between residents and healthcare providers for necessary physical examinations and TB screenings.
- Conduct pain evaluations for residents at the time of admission.
- Communicate relevant information, observed symptoms, and medication adjustments to the supervising team and treatment staff.
- Complete necessary consents and referrals to address the physical, mental, and substance abuse treatment requirements of residents.
- Document changes in medication, PRN usage, and resident responses to treatments accurately.
- Verify residents' medical insurance details with pharmacies and the Business Office.
- Collaborate with the treatment team to provide comprehensive case management for medication and pharmacy services.
- Maintain precise medication logs, including daily administration records and inventory management.
- Educate residents about their medications, including indications and potential side effects.
- Provide information on Medications to Assist with Treatment (M.A.T.) for substance use disorders.
- Ensure the medication room is kept clean and organized.
- Oversee the monthly publication of medication logs.
- Explain the Treatment Policies and Procedures regarding medication to team members.
- Conduct training sessions for staff on medication room protocols and supervise new staff before they assist residents.
- Ensure timely ordering and proper disposal of medications, documenting all related activities.
QUALIFICATIONS
- Must be a Licensed LVN or LPT with experience in residential treatment settings.
- Commitment to completing a minimum of 20 hours of continuing education annually in substance abuse-related topics.
- Familiarity with 12-Step programs and their underlying philosophies.
