Job Title: LVN/LPT - Mental Health Professional

Evolving Treatment Centers is seeking a highly qualified and passionate Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPT) to join our team at our facility in Woodland Hills, CA.

About the Role:

We provide cutting-edge residential and outpatient treatment to adolescents struggling with addiction and mental illness. Our team is dedicated to delivering exceptional care and support to our patients and their families.

Key Responsibilities:
  • Direct patient care and assessment, including reporting of observed changes in mental status and/or physical condition, response to emergencies, and safety risks.
  • Complete documentation in an efficient and timely manner, including nursing assessments, transcription of M.D.s' phone orders.
  • Knowledge of medication administration records, vital sign monitoring, and attendance/participation in treatment planning and professional staff meetings.
  • Call in medications to pharmacy and pick up, provide in-service training to technical staff on medications, documentation, and monitoring of vital signs.
  • Provide ongoing patient education on all aspects of medications and other health issues, including hygiene.
  • Monitor supplies and report needs to Facility Manager, establish a compassionate environment by providing emotional, psychological, and educational support to patients and families.
  • Resolve patient problems and needs by utilizing multidisciplinary team strategies, maintain safe and clean working environment by complying with procedures, rules, and regulations.
  • Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures.
  • Maintain continuity among nurses by documenting and communicating actions, irregularities, and continuing needs to weekend nurse.
Requirements:
  • Valid California Nursing License LVN/LPT.
  • Current CPR/BLS card.
  • Minimum of one year experience as a working nurse. Psych or chemical dependence background preferred.
  • Good communication skills and basic computer literacy.
