About the Role

We are seeking a skilled Registered Nurse (RN) to join our team at Providence Home and Community Care (HCC) as a per diem nurse in our Program of All-Inclusive Care for the Elderly (PACE). This is an exciting opportunity to make a difference in the lives of older adults and their families.

Job Summary

The Clinical Care Specialist will provide professional, independent nursing care for Providence patients, supervising and delegating tasks as needed. You will apply your knowledge of assessing, planning, ordering, giving, and teaching to promote optimal health and independence for our patients.

Responsibilities
  • Provide high-quality nursing care to patients in their homes and in clinics.
  • Supervise and delegate tasks to other healthcare professionals as needed.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
Requirements

To be successful in this role, you will need:

  • A current Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Preferred qualifications include a Bachelor's Degree in Nursing and 1 year of home health RN experience.
