Job SummaryWe are seeking a talented and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a critical role in providing comprehensive care to patients with complex needs. You will work collaboratively with an interdisciplinary team to develop and implement individualized care plans, integrating behavioral health, addiction medicine, and primary care interventions.
Key Responsibilities- Conduct Comprehensive Assessments: Conduct thorough assessments of patients' physical and mental health, substance use history, and social determinants of health.
- Develop Individualized Care Plans: Develop and implement individualized care plans based on patients' needs and treatment goals, integrating behavioral health, addiction medicine, and primary care interventions.
- Administer Medications and Treatments: Administer medications, provide medication-assisted treatment (MAT) for substance use disorders, and monitor patients' responses to treatment.
- Collaborate with Interdisciplinary Teams: Collaborate with an interdisciplinary team, including physicians, therapists, social workers, and other healthcare professionals, to ensure coordinated and holistic care.
- Provide Counseling and Education: Offer evidence-based counseling and psychoeducation to patients and their families, focusing on mental health, addiction, and chronic disease management.
- Monitor Progress and Adjust Treatment Plans: Conduct regular follow-up visits to monitor patients' progress, provide ongoing support, and adjust treatment plans as needed.
- Maintain Accurate Records: Maintain accurate and up-to-date medical records and documentation, adhering to regulatory guidelines and best practices.
- Participate in Continuing Education: Actively participate in continuing education and professional development opportunities.
- Valid Nursing License: Valid nursing license (LPN) in Florida.
- Experience in Integrated Healthcare: Experience working in integrated healthcare settings, behavioral health, addiction medicine, and/or primary care.
- Strong Assessment and Critical Thinking Skills: Strong understanding of mental health disorders, addiction treatment modalities, and physical health.
- Excellent Communication and Interpersonal Skills: Excellent assessment and critical thinking skills, ability to work independently and collaboratively within an interdisciplinary team, and outstanding communication and interpersonal skills.
