Job Summary

We are seeking a skilled Health Navigator to join our team at Roots Community Health Center. As a key member of our staff, you will play a vital role in providing exceptional care and support to our patients.

Key Responsibilities
  • Develop and maintain strong relationships with patients to ensure their comfort and trust.
  • Conduct face-to-face visits and outreach calls to assess patient needs and provide personalized support.
  • Perform comprehensive barrier assessments to identify and address obstacles to healthcare access.
  • Assist patients in enrolling in public benefit programs and accessing medical and social services.
  • Provide health coaching and education on topics such as glucometer training.
  • Collaborate with medical providers and clinical staff to ensure seamless care coordination.
  • Maintain accurate and timely documentation of patient interactions and outcomes.
Requirements
  • 2+ years of experience working in non-profit organizations or community clinics.
  • Comfort working with diverse populations, including African American communities, low-income individuals, and those with behavioral health challenges.
  • Strong organizational skills, attention to detail, and ability to multitask.
  • Effective verbal and written communication skills.
  • Valid California Driver License with a safe driving record.
