The estimated salary for this role is $1,770.60 per week, with the potential for additional compensation through benefits such as 401(k), licensure and relocation reimbursement, and continuing education opportunities.

As a Medical Care Coordinator, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and coordinating with interdisciplinary teams to ensure optimal patient outcomes.

You will also educate patients and families on post-intensive care recovery, medication management, and self-care techniques, and communicate effectively with healthcare team members to facilitate patient progress and optimize outcomes.
