Job Description

The RN Inpatient Care Coordinator will play a key role in coordinating and overseeing the plan of care for selected inpatient populations. This includes promoting effective utilization and monitoring of health care resources and assuming a leadership role with the interdisciplinary team to achieve optimal clinical, financial, and satisfaction of outcomes.

Responsibilities
  1. Coordinates and oversees the plan of care and service of selected inpatient populations across the continuum of illness.
  2. Identifies key problems, strengths, and resources to be addressed in the plan of care which may include over or underutilization of services, premature discharge from an appropriate level of care, need for specific service to improve health status, caregiver fatigue and burden, knowledge deficits, and inability to comply with the plan of treatment.
Requirements
  • Education: Accredited School of Nursing Required: Nursing.
  • Experience: Minimum of 3 years Required: Clinical nursing experience in an acute care hospital and Preferred: Utilization management/quality management experience.
