Job Overview

The Permanency Specialist is responsible for coordinating and delivering essential services and care levels for youth in need.

PRIMARY RESPONSIBILITIES
  • Conducts initial and ongoing evaluations by collecting data from children, families, and relevant parties.
  • Ensures timely initial contact with children within 4 hours of referral and manages subsequent placements.
  • Investigates and suggests potential relative placements, prioritizing them when feasible.
  • Organizes and leads family meetings to facilitate communication and planning.
  • Creates and executes case plans and permanency objectives with families, ensuring timely achievement of goals.
  • Identifies and recommends necessary services for children and families, including mental health support, substance abuse services, respite care, and visitation arrangements.
  • Assists families in building a supportive network of relatives and community resources.
  • Prepares families and children for transitions, reunifications, or other suitable permanency objectives.
  • Provides crisis intervention for families and children in distress.
  • Collaborates with the judicial system, adhering to court protocols and maintaining professional attire.
  • Evaluates ongoing case plan objectives and advises on reunification, parental rights termination, or other permanency alternatives.
  • Prepares documentation for court proceedings, participates in hearings, and ensures compliance with court orders.
  • Maintains accurate and timely written records in the child/family case files.
  • Supports kinship and foster families in sustaining placements and guiding children towards permanency.
  • If adoption becomes the child's permanency goal, assists in completing necessary documentation.
  • Engages in team meetings, sharing vital information regarding children and families' progress, challenges, and solutions.
  • Participates in regular supervision sessions.
  • Collaborates with family support workers, providing guidance and oversight.
  • Supervises and transports children and families as required.
  • Reports critical incidents to relevant authorities within specified time frames.
  • Adheres to organizational policies and procedures, including safety protocols and risk management guidelines.
  • Ensures the protection of clients' rights throughout the process.
ADDITIONAL RESPONSIBILITIES
  • Preferred participation in a minimum of 40 hours of in-service education annually.
  • Other responsibilities as assigned.
QUALIFICATIONS

ESSENTIAL SKILLS AND KNOWLEDGE
  • Mandated reporter status.
  • Training in trauma-informed practices with application in daily responsibilities.
  • Flexibility to work varied schedules, including on-call duties.
  • Familiarity with family-centered practices, genograms, ecomaps, and social histories.
  • Understanding of child welfare systems and regulations.
  • Ability to foster collaborative relationships with families.
  • Knowledge of child and adolescent developmental stages.
  • Effective written and verbal communication skills.
  • Willingness to travel as needed.
  • Demonstrates discretion and independent judgment in performing duties.
  • Capability to work with minimal supervision and manage multiple priorities.
  • Ability to make decisions under pressure and manage stress effectively.
  • Competence in working independently and collaboratively in a team environment.
  • Experience working with diverse populations.
  • Knowledge of and adherence to Universal Precautions.
MINIMUM REQUIREMENTS
  • Age requirement: 21 years or older.
  • Successful completion of background checks and screenings as mandated.
  • FBI fingerprint clearance.
  • Physical ability to lift 30-50 lbs.
  • Possession of a valid driver's license and reliable transportation.
EXPERIENCE AND EDUCATION
  • Minimum of one year of direct service experience in child welfare or a related field.
  • Bachelor's degree in social work or a related field from an accredited institution.
  • Master's degree in social work or a related field from an accredited institution.
CLIENT POPULATION
  • Demonstrates competence in serving clients across various age groups, from infants to adults.
