Job Summary

We are seeking a highly skilled LPN MDS Coordinator to join our team at Mountain Ridge Center in Franklin, NH. As a key member of our interdisciplinary team, you will play a vital role in ensuring the delivery of high-quality patient care and services.

Key Responsibilities
  • Participate in the Resident Assessment Process through the completion of pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs) to ensure accurate patient care and reimbursement.
  • Direct the nursing center interdisciplinary Resident Assessment Instrument (RAI) process while following guidelines for completion, computer encoding, and electronic State transmission of the MDS.
  • Develop individualized resident Care Plans, identifying the needs of the resident and projected outcomes, in accordance with Federal and State regulations.
  • Train new members of the interdisciplinary team on clinical content and electronic completion of the applicable MDS items.
Requirements
  • Must be an LPN in good standing and currently licensed by the state.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility is required.
  • Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures, as well as laws, regulations, and guidelines governing long-term care.
  • Must have computer skills, including Windows applications, and AccuMed experience is preferred.
  • Prior experience as an MDS Coordinator is preferred, and demonstrated proficiency in RAI/PPS processes is required.
  • Must maintain current BLS/CPR certification (excluding ALFs/ILFs).
