We are seeking a skilled and experienced MDS Coordinator RN to join our team.
Job Summary:
The MDS Coordinator RN will be responsible for maintaining and following a schedule of due dates for all MDS, coordinating the completion of the MDS by all disciplines, and reviewing the MDS data for accuracy.
Key Responsibilities:
- Maintain and follow a schedule of due dates for all MDS.
- Coordinate the completion of the MDS by all disciplines.
- Review the MDS data for accuracy and meet with appropriate staff as needed to assure accuracy.
- Complete the MDS and input into the computer.
- Verify that assessments have been transmitted and approved by the State in a timely manner.
- Correct any rejected records and prepare them for re-submission.
- Complete CAAs according to state guidelines.
- Formulate Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
- Schedule and conduct RCP meetings on a regular and timely basis.
- Communicate the RCP to appropriate staff members and notify supervisor if noncompliance by staff is noted.
- Work with all in-house and ancillary departments to assure understanding and compliance with the RCP.
Requirements:
- Registered Nurse, graduated from an accredited School of Nursing and have a current, valid RN license from the North Carolina Board of Nursing and submit proof of license renewal every other year.
- Ability to read, know, and follow personnel, department and facility policies and procedures and adhere to local state and federal requirements.
- Experience with MDS-RAP and Care Planning functions.
- Prefer experience with RUG-IV.
- Prefer experience with MDS 3.0.
