Key Responsibilities- Conduct patient visits and provide routine care under the direction of a registered nurse
- Communicate effectively with the care team, patients, and their families to ensure seamless care coordination
- Participate in interdisciplinary team meetings to discuss patient care plans and outcomes
- Collaborate with other healthcare professionals to provide comprehensive care
- Document patient information and care activities in a timely and accurate manner
- Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
- 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
- Reliable transportation
- Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
