Medical Care Specialist

At Abode Care Partners, we are seeking a skilled Medical Care Specialist to join our team. As a Nurse Practitioner, you will be responsible for delivering evidence-based medical care to seniors and patients with complex needs.
Key Responsibilities:
- Perform examinations and treatments to address chronic and urgent episodic illnesses, minor injuries, and other conditions
- Collaborate with caregivers and other healthcare professionals to develop comprehensive care plans
- Order and interpret laboratory and diagnostic testing as needed
- Ensure patient medical records are up-to-date
- Provide referrals to other healthcare practitioners when necessary
Requirements:
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
