Position Overview:

A Youth Support Specialist plays a crucial role in delivering services that reinforce developmental objectives and promote self-advocacy for children and adolescents enrolled in the HCBS (Home-and Community-Based Services) waiver program. These objectives are outlined in a Service Plan developed in collaboration with your Supervisor. Your primary responsibility is to assist the youth in addressing their needs and achieving the goals established through various interventions that foster independence and personal growth, while also enhancing the family's capacity to support the youth's requirements.

Key Responsibilities:

  • Foster an appreciation for diversity, ensuring that services are attuned to the cultural, social, and individual contexts of each child and their family.
  • Provide tailored educational, advocacy, and support resources that cater to the unique circumstances of the child and their caregivers.
  • Facilitate the child's integration into the family unit and strengthen the family's ability to support them in both home and community environments.
  • Empower families to independently access and utilize resources, promoting long-term self-sufficiency.
  • Deliver comprehensive instruction to enhance understanding of the child's condition or challenges.
  • Engage collaboratively with families and youth to promote a holistic approach to accessing community services and activities.
  • Regularly review and update service plans, ensuring they reflect the evolving needs of the youth and obtaining necessary consents.
  • Collaborate with other service providers to monitor progress, address potential challenges, and streamline referrals.
  • Maintain compliance with all documentation requirements, ensuring timely submission of progress notes, service plans, and logs.
  • Report any incidents to the supervisor promptly.

Service Location & Schedule:

  • Services may be provided in the youth's home or in community settings, emphasizing a comprehensive, integrated approach.
  • Availability for four to six hour shifts during after-school hours, weekends, and non-school days is required.
  • The number of service hours per week will depend on the client's needs and approvals.

Qualifications:

  • Required: Associate's Degree
  • Preferred: Bachelor's Degree
  • Minimum of 1 year of experience in human services
  • Experience working with children/adolescents facing medical, behavioral, or developmental challenges
  • Strong cognitive skills relevant to the position
  • Professional demeanor with a friendly and approachable attitude
  • Excellent documentation skills, ensuring clarity and accuracy

Language Proficiency:
