As a Social Worker, you will be responsible for providing services to address the social and emotional factors that impact our patients' ability to achieve their goals in the Plan of Care. This will involve working closely with other healthcare team members and under the direction of a physician.
Key qualifications for this role include a Master's degree from a Council on Social Work Education-accredited school and a current LCSW license. You will also need a minimum of one year of experience in a healthcare setting, preferably in home care. Strong verbal and written communication skills, as well as good interpersonal skills, are essential for this position.
