We are seeking a skilled and compassionate RN Case Manager to join our team at Andwell Health Partners. As a Hospice RN Case Manager, you will provide high-quality nursing care to patients in their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized plans of care.
Key Responsibilities- Provide skilled nursing services to patients in their own homes, under the direction of a physician's plan of care.
- Develop plans of care with realistic and measurable outcomes, in collaboration with patients and their families.
- Assess and identify patients' holistic healthcare needs, and provide education and support to promote informed decision-making.
- Demonstrate competency in assessment and technical skills, and possess knowledge of pain control and symptom management.
- Current R.N. license in the State of Maine.
- ANA certification in Home Health or Community Health Nursing encouraged.
- Current CPR card required.
- One year of recent clinical nursing experience required.
- A reliable car with insurance.
