Job Title: Licensed Practical Nurse

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our skilled nursing team at EveryAge, a leading provider of long-term care services. As an LPN, you will play a vital role in delivering high-quality patient care and promoting a positive resident experience.

Job Summary:

Under the supervision of a Registered Nurse (RN), the LPN will provide skilled nursing care to residents in our skilled nursing unit. This includes administering medications, performing vital signs, and providing wound care. The ideal candidate will possess excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.

Responsibilities:

  • Provide skilled nursing care to residents in accordance with established policies and procedures.
  • Administer medications and perform vital signs as prescribed by the RN.
  • Assist with wound care, including dressing changes and wound assessments.
  • Participate in interdisciplinary team meetings to discuss resident care plans.
  • Document resident care and progress in the medical record.

Requirements:

  • Current LPN licensure in the state of employment.
  • Minimum of one year of experience in a skilled nursing setting.
  • Geriatric care experience preferred.
  • Basic Life Support (BLS) certification.
