Overview of PACE:

PACE, or Program of All-Inclusive Care for the Elderly, is a holistic care model designed for individuals over 55 years old who require nursing home-level care, focusing on enabling participants to remain in their homes and communities. PACE employs a collaborative, interdisciplinary strategy that addresses all medical and social needs, including prevention, wellness, social engagement, primary care, and access to specialists and other essential services.

The Rehabilitation Specialist evaluates and treats participants upon referral from the Interdisciplinary Team, adhering to the physician's orders. Treatment is delivered through various therapeutic techniques and activities, as well as the use of assistive devices, with the goal of maximizing functional potential and ensuring participant safety. The Rehabilitation Specialist reports directly to the PACE Site Director.

Key Responsibilities:

  • Conduct initial assessments and periodic reassessments for all PACE participants.
  • May assist in completing the Determination of Need (DON) document as part of the PACE enrollment eligibility process.
  • Engage in all necessary functions as a member of the PACE Interdisciplinary Team (IDT), including participation in IDT meetings, family discussions, grievances, service determination requests, and CMS audits.
  • Collaborate with the IDT to create personalized care plans for each PACE participant.
  • Develop and implement individualized treatment strategies with realistic, achievable goals as appropriate.
  • Select and administer relevant objective assessments to identify needs, risks, and changes in status, using this information to formulate an appropriate care plan in coordination with the IDT.
  • Adjust treatment plans and objectives as necessary throughout the care process.
  • Instruct participants and caregivers on safety recommendations, adaptations, exercises, and therapeutic home exercise programs as needed.
  • Ensure timely completion of all required documentation.
  • Participate in the PACE Quality Improvement program as necessary.
  • Supervise Physical Therapist Assistants (PTAs) and Rehabilitation Aides in accordance with the Illinois Physical Therapy Act.
  • Provide training and support to PACE staff as required to meet participant needs.
  • Share clinical insights and recommendations to effectively address each participant's unique requirements within the professional scope of care.
  • Deliver physical therapy interventions and care in alignment with the philosophy and essential functions of PACE, the Illinois State Practice Act, and the APTA.
  • Perform additional duties as assigned.

Qualifications:

  • Reliable transportation is essential.
  • Bilingual candidates are preferred.
  • Valid driver's license and current automobile insurance at PACE organization-specified levels are required.
  • A degree in Physical Therapy from an accredited institution is mandatory.
  • A minimum of 2 years' experience as a PT is preferred.
  • Experience in Inpatient Rehabilitation, Day Rehab, or Long-Term Care settings is advantageous.
  • At least one year of experience working with the frail and elderly population is required.
  • Must hold a valid Physical Therapist license in the state of Illinois.
  • Certification in CPR / BLS by the American Heart Association is required.
  • Maintenance of a valid driver's license and automobile insurance is necessary.
