Geriatric Care Specialist

We're seeking a skilled Geriatric Care Specialist to join our team of independent Physical Therapists. As a key member of our team, you'll provide evidence-based geriatric care to older adults in their homes, helping them maintain their independence and quality of life.

We're looking for a compassionate and hard-working Physical Therapist who is passionate about geriatric care and has a strong desire to develop their skills and achieve clinical excellence. As a Geriatric Care Specialist with FOX Rehabilitation, you'll have the opportunity to work with a variety of patients, each with unique needs and goals. You'll be responsible for delivering proactive and evidence-based therapy to help older adults maintain their strength, mobility, and independence.

Key Responsibilities:
- Provide geriatric house calls to older adults
- Deliver evidence-based therapy to help patients maintain their strength, mobility, and independence
- Work collaboratively with patients, families, and healthcare teams to develop and implement personalized care plans
- Participate in ongoing education and professional development to stay up-to-date on the latest geriatric care practices
Requirements:
- Valid Physical Therapy license in the state(s) of practice, or eligibility to apply
- Degree from an accredited physical therapy program
- Basic computer literacy skills
- Current CPR certification
- Integrity, compassion, and enthusiasm
