Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our team as a Professional Nursing Care Coordinator. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and coordinating services to ensure seamless delivery of medical services.

About the Role:
The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals. You will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare providers to ensure comprehensive care.

Key Responsibilities:
  • Manage patient flow by preparing patients for examination, assisting healthcare providers during exams, and ensuring accurate lab requisition slips and instructions for follow-up care.
  • Assess patient vital signs, reason for visit, and medical history to develop a comprehensive understanding of their needs.
  • Establish rapport with patients and families, providing emotional support and education on self-care and treatment options.
  • Develop and implement educational plans based on patient diagnosis, health status, and quality measures.
  • Collaborate with other healthcare professionals to ensure effective coordination of care and referrals to support services.
  • Provide medication administration and monitoring, ensuring adherence to provider orders and patient safety protocols.
  • Document patient information accurately and efficiently, maintaining confidentiality and adhering to HIPAA guidelines.


Requirements:
To be successful in this role, you must hold a current RN license in the State of Illinois, possess BLS certification, and demonstrate strong clinical and interpersonal skills. A degree from an accredited School of Nursing is required. We also expect you to be customer-focused, dependable, punctual, and able to motivate staff while maintaining a positive attitude.

Work Environment:
This role will work within the So IL Health Care Foundation's healthcare network, which operates according to the highest standards of patient-centered care. The successful candidate will be part of a dynamic team committed to delivering exceptional patient experiences and outcomes. The normal work hours for this position are Monday through Friday, day shift, unless otherwise specified.

Salary Information:
The estimated annual salary range for this role is $65,000 - $80,000, depending on experience and qualifications.
