Job Description:

In the role of a Patient Navigator at Brockton Hospital, you will play a pivotal part in ensuring seamless care transitions for patients with Substance Use Disorders. This involves collaborating with both internal and external entities to remove barriers to care and provide patients with access to appropriate medical and social resources.

You will work closely with patients to address their needs, link them to relevant services, and facilitate smooth communication between healthcare providers. This role requires excellent interpersonal skills, empathy, and the ability to build strong relationships with patients and families.

We are looking for an individual who possesses a strong understanding of addiction treatment, healthcare systems, and community resources. You should be able to exercise good judgment, communicate effectively, and demonstrate compassion and understanding when working with individuals dealing with addiction and behavioral health disorders.

The ideal candidate will possess strong problem-solving skills, be able to multitask under pressure, and have excellent written and verbal communication skills. Proficiency in Microsoft Office and basic computer skills are also essential.

This is a full-time position requiring 40 hours of work per week. We offer a competitive salary, estimated to be around $65,000 - $80,000 annually, depending on experience and qualifications.
