Job Summary

We are seeking a highly skilled Care Coordination Specialist - Addiction Medicine to join our team at Brockton Hospital. This is a full-time, 40-hour/week position that requires excellent communication and interpersonal skills.

In this role, you will be responsible for coordinating care for patients with Substance Use Disorders (SUD) by linking them to appropriate medical and social resources. You will work collaboratively with internal and external entities to remove barriers to care and ensure smooth transitions for patients/families.

The ideal candidate will have a strong understanding of addiction and addiction treatment, as well as experience working in healthcare systems and community resources. A Medical Assistant Certification is required, and an Associate's degree is preferred.
