We are seeking a skilled Registered Nurse to join our team as a Care Coordinator. In this role, you will work closely with our healthcare team to provide high-quality care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities:
  • Assist providers with care coordination activities, including benefit determination and therapy coordination
  • Collaborate with primary care teams to identify and respond to patient changes in condition
  • Document information and activities in the Electronic Medical Record (EMR)
  • Participate in facility-based therapy discussion meetings and CRM meetings
  • Assist with advance care planning and initiating discussions with patients and families
Requirements:
  • 1+ years of experience with using an Electronic Medical Record
  • Valid Driver's License with access to reliable transportation
  • 1+ years of experience in post-acute care, such as long-term care
  • Demonstrated ability to work with diverse care teams in a variety of settings
