About the Role

We are seeking a skilled Hospice Licensed Social Worker to provide counseling and casework services to patients and their families in Worcester territory, working 40 hours per week.

Key Responsibilities
  • Provide counseling and casework services to patients and their families, focusing on the management of the psychosocial status of the patient/family experiencing terminal illness.
  • Render counseling within the context of the physical, emotional, and spiritual needs of patients and their families during the last phase of life, including grief recovery process.
  • Collaborate with healthcare teams to ensure comprehensive care and support for patients and their families.
  • Develop and implement individualized care plans to meet the unique needs of patients and their families.
  • Provide education and support to patients and their families on end-of-life care, advance care planning, and bereavement.
Requirements
  • Master's Degree from a school of social work accredited by the Council on Social Work Education; or Bachelor's Degree in Social Work with a minimum of 2 years' experience in Hospice or PC; or Bachelor's Degree in Psychology with a minimum of 3 years' experience in Hospice or PC.
  • Licensure Required: Licensed LCSW or LICSW to practice in Massachusetts.
  • Experience Preferred: Minimum of one year experience in a health care setting.
