Pinnacle Home Care Inc. - Occupational Therapist Job Description

We are seeking a dedicated and compassionate Occupational Therapist to join our team of skilled home healthcare providers at Pinnacle Home Care Inc.

Job Summary

The Occupational Therapist is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

Responsibilities
  • Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity.
  • Evaluate the home environment for hazards or barriers to independent living and identify equipment needs.
  • Participate in the development of the total plan of care and communicate with referring physicians and other healthcare professionals.
  • Develop treatment programs and establish goals for improved function.
  • Teach new skills or retrain patients in daily activities lost due to illness or injury, in accordance with organization policy.
  • Fabricate splints and instruct patients in the use of adaptive equipment to improve function.
  • Train patients in the use of prosthetic and/or orthotic devices.
  • Maintain clinical records, clinical notes, and reports to physicians any changes in patients' conditions.
  • Submits these records in accordance with organization policy.
  • Maintain contact/communication with other personnel involved in patients' care to promote coordinated and efficient care.
  • Attend and contribute to in-services, case conferences, and other meetings as required by organization policy.
  • Identify patient and family/caregiver needs for other home health services and refer as appropriate.
  • Instruct patients' family/caregiver and other healthcare personnel in patients' treatment regimes when indicated and appropriate.
  • Supervise certified occupational therapy assistants according to organization policy and state regulations.
  • Participate in discharge planning for patients.
Qualifications
  • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Licensed to practice as an OT in the State of Florida.
  • Two years of clinical experience as an occupational therapist.
  • Current BLS Certification.
  • Demonstrates good verbal and written communication and organization skills.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • This position requires that the employee is willing and able to meet the physical requirements as necessary.
