Department OverviewWe're passionate about understanding what matters to people to ensure the delivery of Real-Person Care. Our Care Solutions, Analytics, Implementations, Clinical and Business Operations, and Learning and Development teams work together to address real-life healthcare needs.
As a RN Care Manager, you'll be the clinical bridge between the professional staff of the entire care continuum or in the community. You'll monitor and evaluate the effectiveness of care coordination plans, identify gaps or barriers in treatment plans, and provide education to fill those gaps.
A Day in the Life- Perform care coordination within the scope of licensure for members with complex and/or chronic care needs.
- Conduct assessments to identify individual needs and develop a specific care coordination plan.
- Communicate with care providers, monitor utilization trends, and intervene as necessary to ensure patients are connected with available resources.
- Interface with Medical Directors and Physician Advisors on care coordination treatment plans.
- Provide patient/family/caregiver education about treatment, self-management, and medications.
- Document completed tasks and progress notes in the care coordination system.
You'll be accountable for tracking and reporting daily metrics and productivity measures to assess performance and support data-driven decision-making. You'll ensure an efficient and effective holistic approach for patients/families/caregivers across the care continuum.

If you have a current, unrestricted RN license and experience in critical care, population management, or acute care, we'd love to hear from you.
